Visit our about us pageReferrals can be made for adults of any age who are on their dementia journey. Referrals into the service can be made through Memory Clinics, a GP/health professional or you can also self-refer on this website.

Yes, if the person has been diagnosed with dementia or is concerned about their memory, we are happy to accept referrals from friends and family.
No, we offer information, advice and signposting through appointments and community sessions.
No, everything Bradford Dementia Hub offers is free of charge.
The appointments take place via phone call or home visit, we also run community sessions at our venues across Bradford. Find out about our sessions on our Session and Events page.
We cannot accept a referral for someone living outside of Bradford, but we will work to signpost you to your local support in your area.
Yes, we will work with you to identify what your communication needs are and ensure interpreters are available for your appointment and any adjustments are made
Yes, we will support by making the referrals where you need us to, supporting to communicate needs with the services we are referring to and following up on any pending referrals.
